Registration instructions:  

To solidify your spot for the Collins Hockey Camp in Tempe, Arizona please complete the following:

1. Print and fill out the Registration, Release of Liability and Consent to Treat forms listed below.
   
   
2. Make a $200 deposit by May 25th  - [Full amount $395]
   Make Checks payable to Collins Sport Enterprises
   
   
3. Remainder of fee ($195) will be due June 20th (by mail), or 1 week prior to camp, or at Check-In. 
   
   Players will not be allowed to participate in any CHC on or off ice training sessions
   until all forms have been filled out and handed in, and the fee is paid in full.
